What you provide
- Food production license application: must be filled in truthfully / printed or filled in clearly / stamped with the company seal / and signed by the legal representative or person in charge
- Copy of business license: must be stamped with the company seal / and the business scope of the business license must include the food categories produced / and be within the validity period
- Floor plan of the surrounding environment of the food production and processing site
- Floor plan of the food production and processing site (layout plan of each functional area)
- Process equipment layout drawing (must be marked to scale / consistent with the actual layout)
- Food production process flow chart (key control points must be clearly marked)
- List of main food production equipment / facilities: main production equipment / facilities must be listed / the list must be consistent with the equipment layout drawing
- Personnel information: identity information and qualification certificates of full-time or part-time food safety professional and technical personnel / food safety management personnel
- Food safety management system: including raw and auxiliary material acceptance system / production process control system / factory inspection system / nonconforming product management system, etc. / must be documented and operable
- Trial-produced food inspection report: inspection report of trial-produced food (may be self-inspected by the enterprise or issued by a qualified food inspection agency)
- Product execution standard text: the national food safety standard / industry standard or enterprise standard text implemented for the food produced
- Supplementary materials for special categories (depending on product category)
- Special foods (health foods / foods for special medical purposes / infant formula foods, etc.): production quality management system documents appropriate to the food produced / and relevant product registration and filing documents must be submitted
- Food additives: food additive production license application / equipment layout drawing / process flow chart / equipment and facility list and personnel system must be submitted (requirements are basically the same as food production / but must comply with the specific requirements of the additive variety)
